Form fields are missing labels

Every box, checkbox or button in a fillable form needs a label that says what it is for, such as "First name".

Who it affects

People who use screen readers or fill in forms with a keyboard.

Why it matters

Without a label, a screen reader just says "edit box". The person has to guess what to type, or cannot complete the form.

How to fix it

  1. In Adobe Acrobat Pro: open Prepare Form, then right-click each field and choose Properties. Enter a clear description in Tooltip.
  2. Match the tooltip to the visible label, such as "Date of birth (MM/DD/YYYY)".
